Why Were Separate Institute Exams Abolished?
Before NEET-SS, candidates had to appear for AIIMS, PGIMER, JIPMER, SGPGI, and individual deemed university exams separately β multiple application fees, multiple travel costs, and seat-blocking (securing seats in 4 institutions and vacating 3). The government introduced NEET-SS as a single-window exam to standardise admissions. Today, all superspecialty admissions go through exactly two gates: NEET-SS (most colleges) or INI-SS (AIIMS, PGI, JIPMER, NIMHANS, SCTIMST). Appearing in both maximises your chances.

Typical Career Progression
From MBBS to fully practicing superspecialist β understanding the total investment helps you plan finances and expectations.
Year 1β5.5
MBBS
4.5 years + 1 year internship. Foundation of everything. NEET-UG rank determines college. Government MBBS = βΉ5,000β50,000/yr fees. Private = βΉ10β25L/yr.
Year 6β9
MS / MD / DNB (Postgraduate)
3 years postgraduate residency. The working years β 80 hr weeks, βΉ40,000β75,000/mo stipend. NEET-PG rank is everything. This is where burnout begins if not managed.
Year 9β10
Post-PG Gap / Preparation Year
Most candidates spend 6β18 months preparing for NEET-SS after MS/MD. Work as senior resident or tutor during this time. Average income: βΉ60,000β1L/mo. This is the financial crunch period.
Year 10β13
MCh / DM (Superspecialty)
3 years. Stipend rises: βΉ75,000β1.2L/mo at AIIMS/PGIMER, βΉ50,000β80,000 elsewhere. Sub-specialty training, operative exposure, research output, and network building. The most formative clinical years.
Year 13+
Practice β The Payoff
Private practice, corporate hospital, or academia. Starting salaries: βΉ2β8L/mo for most superspecialties. Cardiology, neurosurgery, HPB, and urology can reach βΉ10β25L/mo within 5 years. Total education investment: 13 years.

What the Toppers Know
Practical strategy for cracking NEET-SS and maximizing your superspecialty career β things no one tells you officially.
Appear in Both Exams
NEET-SS covers most colleges; INI-SS covers AIIMS, PGIMER, JIPMER, NIMHANS, and SCTIMST. Both use different question styles. Appearing in both doubles your seat pool. Most candidates sit only NEET-SS and miss INI-SS seats β a strategic mistake.
Target 2nd and 3rd Choice Fields
Cardiology, neurosurgery, and HPB are glamour fields with 1-in-20 odds. Rheumatology, pulmonology, critical care β same salary ceiling, 1-in-4 odds. Pick the field with the best balance of interest and probability of getting in.
Government > Private for Training
Government MCh/DM: βΉ75K stipend, 0 fees, massive volume. Private MCh: βΉ30β50K stipend, βΉ5β30L fees, less operative exposure. Government training produces more competent surgeons on average. The "prestige" trap is real in private colleges.
DNB Is Now Equal β Use It
Post-2019 reform, DNB holders are eligible for almost all NEET-SS groups. DNB General Surgery β MCh Urology, HPB, Vascular β fully allowed. Many candidates wrongly believe DNB blocks superspecialty. It doesn't.
Start Prep During Final Year PG
Most candidates waste 12β18 months after MS/MD scrambling to start prep. Start dedicated reading in your 3rd year PG. The overlap is hard but it compresses your career timeline by 1β1.5 years β worth βΉ15β25L in lost earnings otherwise.
Fellowship After MCh = Multiplier
HPB fellowship at Germany / Australia / Singapore after Indian MCh = 3β5x salary negotiating power. Even 6-month observership changes your CV permanently. Plan for this during MCh itself β apply in year 2 of your MCh, not after you finish.
Check Institute-Specific Eligibility
AIIMS MCh seats in some specialties prefer MS over DNB. PGIMER Chandigarh has unique requirements for a few DM programs. Always read the official notification β not WhatsApp forwards. Eligibility rules change annually.
Negotiate Using DMFPI Data
After MCh/DM, most hospitals offer below-market rates. Use EMERGE's salary benchmarks (DMFPI) to know P50 and P75 for your specialty before your first negotiation. Saying "I know the market rate for MCh Urology in a corporate hospital is X" changes the conversation.
Know Your Worth Before You Sign
After years of training, don't accept the first offer. Check salary benchmarks for your MCh/DM specialty across hospital types before negotiating your first consultant contract.
